To efficiently begin the process of obtaining therapy with us it is helpful to have new clients complete our new-client packet prior to the initial appointment. The new-client packet provides us with important information regarding your child’s development and history. (Links to forms below.)
You will receive information regarding our policies and procedures, and your privacy rights under law. It is always helpful to receive copies of reports conducted by other healthcare professionals including, but not limited to, neuropsychological testing, audiological testing, and academic testing. This information will allow us to construct a complete picture of your child so we may develop the best possible treatment plan. Completed parent packet forms may be brought to the initial visit or emailed to [email protected].
